Operational Members should be certified in BLS (Basic Life Support) or higher and medically trained.
Please note level and PROVER (in CAPS) with the date of the expiry of your Certificate (i.e. First Aid & HCP Level CPR. RED CROSS. Expires: 1 December 2018). If Expired write NONE / EXPIRED etc.

What was the date you joined ERT SAR (if not sure - the date you paid your application / membership fee.) This helps with seniority and time in service for promotions and operational capacity etc.

ERTSAR Membership Declaration & Agreement

● Will attend training regularly and participate actively in operations and team activities

● Will not attend training or operations while impaired (alcohol or drugs)
● Will maintain professional behaviour at all times, including in uniform and in public
● Will not smoke or act inappropriately in uniform or around the public or children
● Will use issued equipment and PPE properly and follow all safety requirements
● Will maintain confidentiality of incidents, team information, and personal data
● Agree to media/photo release for training, operations, and promotional purposes
● Agree to provide accurate information and update the team if anything changes
● Commit to active membership for at least 2 years (renewable)
●Acknowledge and accept the risks associated with emergency response activities
